The purpose of this summary is to provide you with a general overview of the employee benefits offered by Integrity Technology Solutions to its employees. It is not meant to replace actual benefit plan documents, forms, summaries, or contracts which will always take precedence over this document. Please contact our human resource department for copies of original plan benefit documents.

Health Insurance

• Major medical coverage with low deductibles and total out-of-pocket expense for you and your family.
• Large PPO network of doctors and hospitals throughout the state of Illinois.
• Low doctor s office co-payment not subject to your deductible means you pay a small flat fee each time you see your doctor.
• Company pays 50% of your premium including premiums to cover your family members.

Prescription Drug Card

• You pay a small flat fee for your prescription drugs.
• Fee is not subject to your deductible.
• 50% of premium paid by the company.

Short Term Disability

• Pays a portion of your wages for the first 90 days should you become disabled.
• Maternity included as a short term disability.
• 50% of the premium paid by the company.

Long Term Disability

• Extends benefits if your disability lasts longer than 90 days.
• Benefits paid up to 5 years.
• 50% of the premium paid by the company.

Term Life Insurance

• $15,000 term life policy meant to help your family cover funeral expenses on your behalf.
• Accidental Death feature doubles the benefit amount.
• 50% of the premium paid by the company.

Vision Discount Plan

• Discounts for prescription lenses at select vision care providers.

Direct Deposit

• Have your payroll check deposited to one banking or brokerage account or split between multiple accounts.

401k Retirement Plan

• Save for retirement with tax deferred dollars.
• Multiple investment options.
• Web site allows you to make investment changes and view your account daily.

Health Club Membership

• Group discount at Four Seasons Health Club in Bloomington.

Dry Cleaning Service

• Group discount on your dry cleaning.
• Pickup and delivery at our office.

Paid Holidays

• 5 major holidays plus the day after Thanksgiving.
• Early closing on Christmas Eve and New Years Eve.

Paid Vacation Time

• Earn 2 or 3 weeks of vacation time depending on your length of service.

Paid Sick Time

• Accumulate up to 20 days of sick time to cover unexpected medical absences for you or to care for an immediate family member.

Tuition Reimbursement

• Tuition spending allowance for any approved college or continuing education course work relating to your occupational field.
